These Terms of Service ("Terms") govern your access to and use of the website, mobile applications, and services provided by RevivaGo LLC ("RevivaGo," "we," "us," or "our"), including mobile intravenous (IV) hydration and vitamin therapy services delivered to your location in the East Valley, Arizona area.

By accessing our website at revivago.com, creating an account, or booking a service, you agree to be bound by these Terms. If you do not agree to these Terms, do not use our services.

These Terms constitute a legally binding agreement between you and RevivaGo. Please read them carefully before using our services.

Eligibility

To use RevivaGo services, you must:

  • Be at least 18 years of age.
  • Be located within our service area in the East Valley, Arizona (including Queen Creek, Gilbert, San Tan Valley, and Mesa) at the time of service delivery.
  • Provide accurate and complete medical history and personal information during the intake process.
  • Be capable of providing informed consent for medical treatment.

Individuals under 18 may receive services only with the presence and written consent of a parent or legal guardian. RevivaGo reserves the right to refuse service to anyone who does not meet eligibility requirements or whose medical screening indicates contraindications.

Description of Services

RevivaGo provides mobile IV hydration therapy and vitamin infusion services delivered to your home, office, hotel, or other approved location. Our services include but are not limited to hydration therapy, vitamin and nutrient infusions, immunity support, athletic recovery, and wellness treatments.

All clinical services are performed by licensed healthcare providers, including Nurse Practitioners (NPs), Registered Nurses (RNs), and licensed paramedics, operating under physician oversight in accordance with Arizona state law.

RevivaGo services are wellness-oriented and are not a substitute for emergency medical care, primary care physician visits, or hospital-based treatment. If you are experiencing a medical emergency, call 911 immediately.

Booking & Scheduling

Appointments may be booked through our website at revivago.com. When you book an appointment, you agree to:

  • Provide a valid service address within our service area.
  • Complete the required medical intake form accurately and honestly.
  • Be present and available at the scheduled time and location.
  • Provide a clean, safe, and well-lit environment for your provider to deliver care.

Appointment availability is subject to provider schedules and service area coverage. RevivaGo reserves the right to reschedule or cancel appointments due to provider availability, weather conditions, safety concerns, or other circumstances beyond our control.

Medical Screening & Informed Consent

Before receiving any IV therapy, you are required to complete a medical intake questionnaire. This information is reviewed by a qualified healthcare provider to determine whether services are appropriate and safe for you.

RevivaGo reserves the right to decline service based on the results of your medical screening. If your medical intake raises concerns about contraindications, allergies, current medications, or underlying health conditions, we may recommend that you consult with your primary care provider before receiving treatment.

By booking and receiving services, you acknowledge that you have been informed of the potential risks and benefits of IV therapy, including but not limited to bruising at the injection site, allergic reactions, infection, and vein inflammation, and you voluntarily consent to treatment.

Billing & Payments

Service pricing is listed on our website and may vary based on the treatment selected, add-on services, and promotional offers. All prices are in U.S. dollars.

Payment is collected at the time of booking through our secure payment processor. We accept major credit cards and debit cards. A payment authorization hold may be placed at the time of booking and finalized upon completion of service.

RevivaGo does not bill insurance. Our services are self-pay. You may request documentation of services rendered to submit to your insurance provider for potential out-of-network reimbursement, but RevivaGo makes no guarantee of insurance coverage or reimbursement.

Promotional pricing, discount codes, and special offers are subject to their stated terms and expiration dates. RevivaGo reserves the right to modify pricing at any time, though changes will not affect appointments already confirmed.

Cancellations & Refunds

You may cancel or reschedule an appointment at no charge with at least 24 hours' notice before the scheduled appointment time.

Cancellations made less than 24 hours before the appointment or no-shows may be subject to a cancellation fee of up to 50% of the scheduled service cost.

If RevivaGo cancels your appointment due to provider unavailability or circumstances outside our control, you will receive a full refund or the option to reschedule at no additional cost.

Refunds for services already rendered are evaluated on a case-by-case basis. If you experienced a service issue, please contact us at hello@revivago.com within 48 hours of your appointment.

Account Terms

You may create an account on our website to manage bookings and medical information. You are responsible for:

  • Maintaining the confidentiality of your account credentials.
  • All activities that occur under your account.
  • Notifying us immediately of any unauthorized access to your account.
  • Ensuring that your contact information and medical history remain accurate and up to date.

RevivaGo reserves the right to suspend or terminate accounts that violate these Terms, provide fraudulent information, or engage in abusive conduct toward our staff or providers.

Acceptable Use

You agree not to:

  • Provide false or misleading information in your medical intake or account registration.
  • Use our services for any unlawful purpose.
  • Interfere with or disrupt the operation of our website or services.
  • Attempt to gain unauthorized access to our systems or other users' accounts.
  • Engage in threatening, abusive, or harassing behavior toward RevivaGo staff, providers, or other customers.

Our providers reserve the right to discontinue a treatment session at any time if they determine it is not safe to continue, including situations where the service environment is unsafe or the client exhibits disruptive behavior.

Intellectual Property

All content on the RevivaGo website, including text, graphics, logos, images, and software, is the property of RevivaGo LLC or its content providers and is protected by United States copyright and trademark laws. You may not reproduce, distribute, modify, or create derivative works from any content on our website without prior written permission.

Medical Disclaimers

RevivaGo provides wellness-focused IV hydration and vitamin therapy services. Our services are not intended to diagnose, treat, cure, or prevent any disease. The information provided on our website and by our providers is for general wellness purposes and does not constitute medical advice.

IV therapy carries inherent risks including, but not limited to, bruising, soreness at the injection site, allergic reactions to ingredients, infection, air embolism, and vein inflammation (phlebitis). While serious complications are rare, they can occur.

You should consult your primary care physician before receiving IV therapy if you have kidney disease, heart failure, high blood pressure, or any condition that may be affected by fluid or vitamin intake. Our providers will assess your suitability during the medical screening process, but this does not replace your own physician's guidance.

Results from IV therapy vary between individuals. RevivaGo does not guarantee specific outcomes from any treatment.

Limitations of Liability

To the fullest extent permitted by Arizona law, RevivaGo LLC, its officers, directors, employees, and contracted healthcare providers shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services, including but not limited to loss of profits, data, or goodwill.

Our total liability for any claim arising from or related to these Terms or our services shall not exceed the amount you paid to RevivaGo for the specific service that gave rise to the claim.

This limitation of liability does not apply to damages resulting from gross negligence or willful misconduct by RevivaGo or its providers.

Indemnification

You agree to indemnify and hold harmless RevivaGo LLC, its officers, directors, employees, and agents from any claims, liabilities, damages, losses, or expenses (including reasonable attorneys' fees) arising from your violation of these Terms, your misuse of our services, or your provision of inaccurate medical or personal information.

Governing Law

These Terms are governed by and construed in accordance with the laws of the State of Arizona, without regard to conflict of law principles. Any disputes arising from these Terms or your use of RevivaGo services shall be resolved in the state or federal courts located in Maricopa County, Arizona.

If any provision of these Terms is found to be unenforceable, the remaining provisions shall continue in full force and effect.

Changes to These Terms

RevivaGo reserves the right to update these Terms at any time. When we make material changes, we will update the "Last updated" date at the top of this page and, where appropriate, notify you by email or through a notice on our website.

Your continued use of our services after changes to these Terms constitutes acceptance of the revised Terms.
